covered under our medical plan. Coverage is effective on first day of
employment.

- Plan pays 80% for fillings.
- Plan pays 50% for crowns and inlays.
- Low monthly family premium for dependent coverage (optional).

Short-term Disability:

pays the full cost of short-term disability for every employee.
Coverage begins on first day of employment.
Short-term disability will pay you 60% of your base earnings, up to $750 a
week, for up to 90 days, after satisfying a waiting period.

Long-term Disability:
SyllogisTeks pays the full cost of long-term disability for every employee.
Coverage begins on first day of employment.
This program will pay 60% of your basic monthly salary if disabled.
The employee has a 90-day waiting period. The benefit duration is to
age 65 as you continue to be disabled. The monthly maximum is $5,000.

employee on the first day of employment.
We
have voluntary life insurance that you can purchase for yourself and your

contribute up to the current federal limit each year.
- SyllogisTeks will match 50% of Employee contribution up to 6% of salary.
- Eligible to contribute the first of the month following 30 days of
